Alternaleaf Review UK 2026: Costs, Appointments, Pros & Cons

Alternaleaf has become one of the most visible names in the UK private medical cannabis market, offering an entirely remote service with online consultations, home delivery and a choice between monthly membership and pay-as-you-go appointments.

But is Alternaleaf the right medical cannabis clinic for you?

In this independent Alternaleaf review, we examine its current costs, eligibility process, appointments, prescriptions, delivery and regulatory record, and explain what prospective patients should compare before registering.

Quick verdict: Alternaleaf currently has one of the simpler pricing structures among major UK clinics, with £5-per-month membership or £29 pay-as-you-go appointments. However, prospective patients should also consider its current CQC rating alongside price and convenience.

Alternaleaf states that its £5 monthly membership covers all required online appointments. Its PAYG alternative currently costs £29 per appointment. Medication is additional.

How Much Does Alternaleaf Cost?

This is one of Alternaleaf's strongest propositions.

Under its current membership:

£5 × 12 months = £60 per year in clinic membership fees.

Alternaleaf currently expects four appointments during the first year: the initial process, a follow-up at approximately one month, a six-month review and a 12-month review.

Under PAYG, its published first-year appointment cost is £116.

That makes the £60 membership £56 cheaper on published first-year clinic fees, assuming the current appointment schedule.

Remember that these figures do not include medication.

Alternaleaf advertises medication from around £20 per week and flower starting at £5 per gram, but actual expenditure depends on what is clinically prescribed.

What Is Included?

Both current payment options include personalised clinical care, travel letters and one free medication delivery each month.

Alternaleaf also states that it does not charge repeat-prescription fees.

For someone comparing clinics purely on administrative costs, that simplicity is attractive.

How Does Alternaleaf Work?

The process is online.

First, prospective patients complete a short pre-screening questionnaire. Alternaleaf then requires medical information, including NHS records, before prescribing.

Consultations take place remotely.

If treatment is considered clinically appropriate and a prescription is issued, the prescription passes to a specialist pharmacy and medication can be delivered to the patient's address.

Alternaleaf says appointments are available seven days a week, with weekday appointments extending into the evening.

Who May Be Eligible?

Alternaleaf's current published criteria include:

  • being at least 18;
  • having a diagnosed condition;
  • having been offered at least two conventional treatments;
  • not being pregnant;
  • not having a history of psychosis or schizophrenia.

Previous treatments don't necessarily mean two different medicines. Alternaleaf says they can include interventions such as medication, talking therapy, physiotherapy or surgery.

These are Alternaleaf's screening criteria, rather than a guarantee of prescription or a universal statutory test for medical cannabis.

The ultimate prescribing decision is clinical.

Prescriptions and Medication

Alternaleaf says it has access to more than 100 medications and works with specialist UK pharmacies.

Patients generally receive a month's supply. Following the early follow-up period, repeat prescriptions can be requested between clinical reviews.

An important consideration for consumers is pharmacy/product choice.

Alternaleaf says prescriptions are drawn from a curated list available through its partner Montu UK pharmacy, including both in-house and other products. Patients particularly concerned about product or pharmacy flexibility should therefore investigate the current arrangements before joining.

Alternaleaf CQC Rating

This deserves more prominence than most affiliate reviews give it.

Alternaleaf is operated by Montu Group UK Ltd from its CQC-registered Winnersh location.

Following an assessment in July 2025, the Care Quality Commission rated the service Requires Improvement. The report was published on 27 February 2026.

The CQC reported breaches relating to safe care and treatment, premises and good governance, and stated that a warning notice concerning safe care and treatment had been served in September 2025.

Alternaleaf displays the Requires Improvement rating on its own website.

That doesn't tell an individual patient what their experience will be, but it is material independent regulatory information and should form part of a clinic comparison.

Alternaleaf vs Other Clinics

Alternaleaf's £60 annual membership makes its published clinic fees highly competitive.

Curaleaf currently goes slightly lower with a £50 annual upfront option, while also offering £5 monthly membership. Mamedica's structure involves larger initial charges but some programmes remove clinic fees later.

This is precisely why headline appointment price alone isn't enough.
